I voluntarily consent to receive facial and esthetic services from Eden Facial Studio.

I understand that my treatment may be customized based on my skin analysis, skin concerns, and the information I provide to my esthetician.

I understand that facial treatments may occasionally cause temporary redness, irritation, sensitivity, dryness, breakouts, peeling, swelling, or an allergic/sensitivity reaction.

I understand that individual results vary and no specific result is guaranteed.

I confirm that I have provided accurate information regarding allergies, medications, skincare products, medical or skin conditions, pregnancy, and recent treatments that could affect my service.

I understand that Eden Facial Studio provides cosmetic esthetic services and does not diagnose or treat medical conditions.

I agree to tell my esthetician immediately if I experience unusual pain, burning, or discomfort during treatment. I understand that I may ask for the treatment to stop at any time.

By signing below, I acknowledge that I understand the nature of the treatment and its reasonably foreseeable risks, have had the opportunity to ask questions, and voluntarily consent to treatment.

Skin Analyzer Photography

As part of your consultation and treatment, Eden Facial Studio uses a skin-analysis system that captures images of your face. These images allow your esthetician to evaluate your skin, personalize your treatment, make skincare recommendations, and compare your skin's progress over time.

I understand that these images will become part of my client treatment record and are required for services that include the skin-analysis process.

I consent to Eden Facial Studio capturing, storing, and using my skin-analyzer images for my personal skin analysis, treatment planning, recommendations, and progress tracking.

Training & Education Permission — Optional

I give Eden Facial Studio permission to use my skin-analyzer images and/or treatment photographs for internal staff training and professional education.

This permission is optional and is not required to receive treatment.
